Diabetes, like high blood pressure, weight gain, and many other ailments/conditions are primarily caused by genetics and aging. Eating a balanced diet and avoiding tobacco etc. can offset these but not prevent them.

By the way, I mentioned Menominee County due to the reservation there. Indians (Native Americans) have notoriously high rates of diabetes: Diet, alcohol, and genetics.
Can’t blame McDonalds for that one. There aren’t any on the res. that I know of.
There are also a lot of older folks there, people who retire on the lakes.
Looking at the map above, Asian countires seems to have more diabetics.
The vast south east of the country looks like it has the chicken pox. Washington district looks like it’s on a free diet plan that’s going swell!
